Ames care center fined for violations

DES MOINES, Iowa (AP) - The state has fined an Ames care center for dozens of violations, including the abuse of an elderly resident.

The Iowa Department of Inspections and Appeals filed a 137-page list of problems at the Abington on Grand and fined the home $4,500.

The 79-bed home is owned by American Healthcare Associates of Huxley.

Among the allegations:

-A worker barricaded a female resident in the resident's room by moving furniture against the door. When the woman was found by other workers she was "tearful and anxious. " Inspectors cited the home for not reporting the incident as a case of abuse.

-The same worker was involved in another incident with the same resident in which the worker grabbed the woman's walker with enough force that the woman's head flung back and struck an object. The incident happened after the woman was throwing salt and pepper shakers in the dining room.

-A review of files of 14 residents by inspectors showed the home failed to ensure that six of the residents were properly fed.

-The home failed to provide timely treatment for a resident with multiple skin tears. On two occasions the residents clothes or bed sheets were found bloodied. Staff didn't notify a doctor on either date.

-Staff admitted that they were getting residents out of bed for breakfast as early as 5 a. m. , three hours before the meal was served.

-Residents complained that sometimes it took three to four weeks to get clothes back from the laundry. Some linen closets had no clean towels, washcloths or bed gowns. An employee told inspectors she was the only person hired to do laundry and that she worked one eight-hour shift per week. The home has 52 residents.

-Some residents didn't receive prescribed medicines and nutritional supplements. Lab tests done for other residents were not done in a timely manner and nine residents didn't receive help with their prescribed exercises.

-After one worker tried to block an inspectors' entrance to kitchen storage area the inspector found dead mice on the floor and several pans and pan lids covering the floor next to a mousetrap.

The Abington on Grand also was fined $2,750 last year for various problems.

American Healthcare Associates owns 14 nursing homes in Iowa.

Calls made Tuesday to Russ Hemness, an executive with the company, were not immediately returned.
